Issues when creating new forms

I created a new form with Form Builder 201 extension. When I was done formatting and entering all of my fields, I was unable to select the Form Functions tab to enter my email address, subject & thank you page.

After I clicked finish on the wizard, I uploaded all of my files, I receive an error: "Parse error: syntax error, unexpected '"', expecting T_STRING or T_VARIABLE or T_NUM_STRING in /home/petsit/public_html/service.php on line 5"

This keeps happening with every new form that I try to create, not just on this website. I've attached the php file. Please help!
