Form Builder issues lead to Extension Manager problems

I am trying to uninstall and reinstall the form builder extension because it stopped working. Ever since the form builder stopped working, I now cannot open the Adobe Extension Manager CS5.5. When I try to, nothing happens. However, in my task manager, the program is running under the Processes tab but not under Applications.

I've tried uninstalling Extensions Manager, reinstalled it, nothing changed. Then I uninstalled it and installed version CS6. This program will open but when I tell it to import my previous extensions, nothing happens. When I click on a downloaded extension (.mxp) file, it acts like its going to install the extension but then it gives me this error: "This extension can not be installed, it requires Dreamweaver version 9 or greater."

I have Adobe Creative Suite CS5.5 installed so this error doesn't make sense. I have Windows Vista and I've never had any problems with the Extensions Manager before until Form Builder stopped working. I don't know what to do to move forward. Please help!
