New re-direct issue

New re-direct issue

All of a sudden my pages on several sites are no longer re-directing correctly. After an email is sent the re-direct should go to a file in at the same level but instead the URL is wrong.

here is the redirect code: $GoToPage = "card_created.php"; which should result in:

card_created.php this has worked for a year!

Instead it's redirecting to:
card_created.php

I have upgraded to css form builder 2.01 & HTML 3.0.2

This same problem occurred on another site on another server. I assume the problem is an include. See attached:
