After Update/Insert/Delete, 404 Error

I am relatively new to DataAssist. I am using DW CS5.5 and DA 2.08. There appears to be a form action issue and I am not sure how to resolve.
Delete Page: <form action="rates_Delete.php?ID=<?php echo(rawurlencode($row_WADArates['ID'])); ?>" method="post" name="WADADeleteForm" id="WADADeleteForm">

Update Page: <form action="rates_Update.php?ID=<?php echo(rawurlencode($row_WADArates['ID'])); ?>" method="post" name="WADAUpdateForm" id="WADAUpdateForm">

Insert Page: <form action="rates_Insert.php" method="post" name="WADAInsertForm" id="WADAInsertForm">

After submitting each form, I go to a 404 Page Not Found error. If I refresh the page, then it shows properly with whatever changes were made. What edit do I need to make?
