UE not working when form included

UE not working when form included


I have UE working great on all forms excpet for a form I have <?php include ('myform.php'); ?>

I start by setting up the form in the myform.php page then when all set up I copy the below into line 1 of the page the myform.php is being included in. Im just not getting any emails at all - it is redirecting ok.

header("Location: contact_success.php");

ANother issue i have is that once a form has been added how do I edit the form without having to edit in code mode?
