you have to add those form element names to the remove array:

$remove = array();
$remove[] = "";
$remove[] = "x";
$remove[] = "y";
$removeBegins[] = "hidden";
$removeBegins[] = "submit";
$removeBegins[] = "Register";
$removeBegins[] = "MM_insert";

to show an image in the email, you would need to use a fully qualified URL to the image file in the src tag:

<img src="" />

the code that displays the time in the emal is:

<?php $now time(); ?><?php echo date("n-j-Y"$now); ?>

you can remove that code or change it to:

<?php $now time(); ?><?php date_default_timezone_set('Australia/Perth'); echo date("n-j-Y"$now); ?>

to change the time zone. a list of supported Australia time zones can be found here:

to customize the labels that show in the email, you would need to chage the name s of the form elements, be aware that doing so, however, may have other consequences on your page. it may break other functionality.
