Keeps Bombing Out

I have a standard PHP form I usually throw into basic sites that works well enough. I'd like to try sprucing some of the contact forms up for new sites. Every time I go through all the steps of making presets, fonts, rules, margins, etc.. At some point the application quits out only when I use the form builder.
I'm on a new iMac i7 packed full with Ram so it's not a memory issue. I tried reinstalling it but no luck. It always craps out either during the setting up or when I hit Finish...

What version of Dreamweaver are you using?

What extensions including the version numbers do you have installed?

Extensions, etc.

Dreamweaver CS5 (my 5.5 extension manager is glitchy and I'm working with Adobe on rectifying)...

Design Extender 1.1.1
Data Bridge 1.1.0
Free Tools 1.0.2
Dreamweaver HTML5 Pack 1.2.7

Form builder is a part of Data Bridge, the most current version of Data Bridge is 1.1.3, make sure you uninstall the previous version from the extension manager, then install the newer version that is available from your download center.
