Design Extender - Form Builder Preview Error

I am getting a preview error with my forms after Installing Design Extender. I have uninstalled all the extensions that are included in Design Extender, have deleted the Dreamweaver configuration file, and have tried to uninstall and re-install the extension. Yet the problem persist.

what other extensions do you have installed, including version numbers?

Make sure that Design Extender is the last extension that you install.

Have you tried resting the workspace payout? Go to Window -> Workspace Layout -> Reset '<your layout name>'
