Form Submit NOT sending info via email

Hello I have used universal-email for many, many years without issues... I now have a 2 page form and it's NOT sending ANY info to the TO: email address

I M STUMPED - HELP! I included the files minus images etc

You need to move the universal email code to the form action page and not the page with the form itself.

It should work if you move the UE code to "payment.php"

You are responding to the form post... the form posts to the payment.php page, so that is where the email functionality has to be added.
