Two Search Forms on One Page

Hi, I have two forms on one page. Each works fine when I have just one on the page, but when I have the two on the page I get the following error.
Fatal error: Class 'FilterDef' not found in members_Results_Invoice.php on line 23

I have included the file for your perusal.

The reason for the form not working is that the require_once for the Database Search is below the code that is calling something from that file. If you move this to the top of the page:
<?php
//WA Database Search Include
require_once("WADbSearch/HelperPHP.php");
?>

Then the search should work again.
