Form Builder Does Not Generate working code

I have started from a blank ASP page and created an input form. After I have done that I go to the DW menu and select to link to an SQL data base that is defined. I assign all of the fields and publish the page to the web. When I go to access the page NOTHING HAPPENS. I get a page can not be dispayed.

When I look at the code that Form Builder has generated -- It looks like it generates duplicate data.

ASP is not a supported language for CSS Form Builder, so it will not generate working code for ASP.
