form elements not depicted in client validation

hi - i built a form by hand and want to add client validation to it.
the page has two forms, and was i able to use the form builder->client
validation - to validate one of them ( the footer form see attached).
the other form - a sidebar form) -
when attempting to validate the elements therein, are not
listed in the form validation tool. since no form elements were listed,
i therefore cannot validate each of them.
any ideas why this might be?

change the form element so the name and ID are the same. Change:

html:
<form name="sidebarform"  id="sidebardatacapture" method="post" action="/gdform.php">




to:

html:
<form name="sidebarform"  id="sidebarform" method="post" action="/gdform.php">
