Form Builder Error

Form Builder Error

This is the second time I am trying to use this extension and am getting this error on shared.js file Please see attached screen shot.
This happens no matter which form design is used. I have deleted the winfilecashe and restarted Dreamweaver but I get the same error.
Appreciate your help.

Try opening the WebAssist Site Tools floater before running the Form Builder Wizard. You can open this floater by going to the Site Menu in Dreamweaver and selecting WebAssist Site Tools.

If this doesn't work, please provide some more detailed information on what steps you are following to get the error and at what point you are getting the error. Also, which version of Dreamweaver you are using and what Operating system.

That didn't work. This happens once I have selected the form and the design. For example I select the contact form then select the sharp design and then I press finish. That's when I get the error. I am using Windows Vista, Dreamweaver CS4 and the latest version of CSS Form Builder.

I fixed this problem. I think the extension was corrupted somehow. I removed it and re-installed the extension and the problem went away.
