Form Dilemma

I can build a nice looking form with Form Builder (see attached jpeg "Screen Shot Good Form") but when I do anything else to the file like reapply a DW Template it corrupts, see attached "Screen Shot BadForm." I had added an html Site Map to the template. I know this isn't a lot for you to go on so let me know what further information you need from me. Hoping there is something i am forgetting to do to save, etc. Or is there a work around involving saving css files and reapplying to corrupt file. My limited experience with code really shows in a situation like this. Thanking you in advance.

It looks as though you are having some interference with the css that the forms are using. If this is the case you should be able to just move the include lines for the css of the form down below the css references in your template. If you have a link to the live page, or both versions of the form this would be best. I could take a look and try some things with the css on these pages to see what you may need to do to get the form looking correct.
