So this is not for installing a search engine to search your own database, correct? It's only to apply to your webpages?

It applies a Yahoo search to your website. The results are not branded by Yahoo and are fully customizable.

If I wanted a search page/engine to search my mysql db and return results only from that, what would be the best way to go? D/A or this?

To search your Database, you will need to use DataAssist Search. Search for BOSS Searches live pages that are indexed by Yahoo Search Engine.

So, will it return results from sites other than your own, say a competitors site?

If your site doesn't return good results from search engines such as google and yahoo, does that also mean that search for boss on your own site won't return good results either?

The only results that you'll get are from your site. I haven't seen it fail to report a result yet.
I guess it really depends on the content on your site and the actual search criteria that you post.

Example: The site is focused on flowers and gardening.
Search Criteria: politics

The two have nothing in common and I would expect the result set to be empty.

The results that are returned are only returned from the domain that you have entered in the extension.
