Checkout works until I change test request to FALSE

My checkout works fine in test mode. As soon as I change test request to FALSE, without changing anything else, I get the checkout failure page. My Authorize.net account is not set to test mode. I have had a working shopping cart before but can't get this one to work. Here are my checkout and confirmation pages in text format. Thanks for you help.

seems to be working now

It looks like the address verification was causing the failure, and it apparently just accepted the transaction without checking in test mode.
