JavaScript error after patch

I'm getting the following error after installing the recent support fixes when I try to Edit Form Contents. And then Form Builder just hangs.

While executing onLoad in WAFB_EditFormContents.htm, the following JavaScript error(s) occurred:

In file "WAFB_EditFormContents":
initializeUI is not defined

Try deleting the Dreamweaver cache file from the users configuration folder:
common_installation_issues.pdf

Didn't work

I did try deleting the cache file and it caused a bunch of new errors when I opened Dreamweaver. I'm under a deadline and had to do something, so I decided to remove all WebAssist extensions - those errors no longer come up. I reinstalled Form Builder and am now reinstalling the ones I'm currently using - what a pain.
