Setting up second form

I'm having a major issue with setting up a form page i created. I have one form that is working and I wanted to create another from in the same directory.

I don't seem to be doing this right, because thinking is working not even the button which maybe is the issue but I cant figure it out.

Any help would be GREATLY appreciated.

Your page has two form on it. The first one does not have any check boxes in it, and can probably be removed.

For the second one, you will ned to change the form tag from:
<form action="#">

<form action="" method="post">
