check New User Name failure

Getting function failure. - using the MySQLi check new user name function which edits fine, then fails when attempting to execute and also fails when attempting to re-edit the code via webassist.

I have the identical "add & check" routine running on another database with no issues.
I have attached the php file, and screen captures of the browser error and the dreamweaver re-edit attempt.

Can you assist me on what is wrong.

I've updated the extension to fix the error when re-applying. Just reinstall and it shouldn't happen any more.

The runtime issue is a code placement issue. This can happen occasionally with Dreamweaver. Just move line 17 up above with the other includes and that error should go away. It is a library used in the code but it was placed below the code that references it and that caused the undefined error.
