Validation

This page has used to use another extension that does not seem to work anymore as the page is getting past validation without even checking. Wondering if you can fix this page up to work with Webassist.

What type of validation do you want on what fields?

Which are required? Which have minimum values? I'll need to know that to finish adding validation. I've added validation on the first few, but I'm not even sure if they were supposed to be required.

Look at the submit button and body onLoad, it's pretty self explanatory
