Data Assist Form with File Upload has stopped working

I have a form to add a news article and photo to a website. It has been working well for some time but has all of a sudden stopped working.

I don't get any errors on either insert or update however the file does not upload.

I have gone back in and have opened the wizards for both the insert and upload and have saved again to see if any files needed updating, however, this hasn't helped.

I am getting an error in the developer view though

Uncaught TypeError: $(...).h5Validate is not a function
at Insert_Basic_Default_Validate (AddNews.php:267)
at HTMLDocument.<anonymous> (AddNews.php:270)
at j (jquery.min.js:2)
at Object.fireWith [as resolveWith] (jquery.min.js:2)
at Function.ready (jquery.min.js:2)
at HTMLDocument.J (jquery.min.js:2)

I updated the folder permissions for the upload folder to give write access and my insert worked.

Maybe it has something to do with the file you are uploading. Maybe it is too large for your php settings. It clearly works for me.

Also you should add this code to the top of your plugin file:
[pgp]
<?php
if (!class_exists("WA_Include")) die();
?>
[/php]

That will prevent people from accessing the plugin directly before logging in.
