Insert captcha not working

I'm trying to insert captcha on a contact form. When I click in the cell of the table that I want to put the captcha, launch and adjust the interface (font selection etc) and click enter, the captcha appears but the entire form is gone??? I've attached the page that we are attempting to insert to.

That is odd. I was able to open the page and add CAPTCHA to an empty cell without any issues. Maybe use code view to make sure your cursor is in the correct location and try again. Since I'm unable to reproduce the problem I'd need to set up a screen sharing session to debug any further. If you continue to have problems you can post back with a US phone number and good time to reach you and I can set up a screen sharing session and figure out what is going on.
