Cannot modify headers

I've reviewed the many many threads with this issue. I've done a find all for "?> ", minding the extra space, and found no occurrences.

I've ensured the BOM is included, and am still getting the error.

What is on line 164 of agency-duescalc.php? Can I get a copy of that page? That will help me see what is wrong.

You have an html comment on line 163:

<!--**/Email file is in root directory, called email-invoice.php -->

Comments in php need to use the php comment syntax:

<?php // Email file is in root directory, called email-invoice.php ?>
