Form doesn't work on ios devices.

This form isn't working on ios devices such as iphone and ipad. I'm getting complaints from customers that the form isn't working for them.

It's the same form that i had an issue with in my previous post here: http://www.webassist.com/forums/posts.php?id=39500
You have all the files in that post.

All of your form elements are contained within a single <label> tag. You need to add a separate <label> for each <select>
