upload file not working

Hi all,
I tried everything, but I cannot make "upload file" to work.
I have a form with a file field.
Here is what I have done:
1- selected file field in design view
2- dropped "my_file_field" from bindings panel over the file field (there are other items related to the file field that are referenced witha lighning icon; I ignored them; but would be nice to know how to use them; there a re no info about that on the help system))
3- added the web assist > file manipulation > upload file server behaviour and set with the values specified in the attachments (private)
4- edited the Insert Single record server behaviour and pointed the file field to the form file field

TESTING the page result: record inserted but file field is empty and no file has been uploaded.
NOTE: upload file folder ("uploads") has this file permission: 777.

If I add WA validation (file extension) to the field, it fails even when the file extenion is right.

Here is the debug info displayed when validation fails:

Status object: WA_UploadResult1
statusCode : -1
errorMessage :
clientFileName :
fileExtension :
serverFileName :
serverSimpleFileName :
serverDirectory :
contentType :
fileWasOverwritten :
fileWasSkipped :
fileWasRenamed :
fileSize : -1
isImage :
imageWidth : -1
imageHeight : -1
WA_UploadResult1_1:
clientFileName :
fileExtension :
serverFileName :
serverSimpleFileName :
serverDirectory :
contentType :
fileSize : -1
imageWidth : -1
imageHeight : -1

How can I make it to work?
TIA
tony

Ok.
I got it working.
The enctype="multipart/form-data" was missing on the form definition.
Now it works as expected.
