form not showing when added to template

generated form using form builder within a page generated from a framework theme, nothing shows. (step 1)

to make sure every thing ok, generated form within normal blank page, all ok, (step 2)
copied/renamed file from (step1) still the same (step 3)

swapped the include around (cms /sb) still no joy. (step 3))
added php error handling none reported

keep getting message, click ok/press esc se attachment
any ideas

To fix the error, try deleting your winfilecache .dat file from your users configuration folder while Dreamweaver is closed.

I can't imaging why the form isn't displaying on your page, but if it works when you apply it to a different page, then try adding it to your existing page as a framework builder plugin and then it should work.

cache delete - no more errors

used plugin minor tweaks required the wise success
