WA_Receipt missing customer billing address and shipping address

Greetings to all at WebAssist. I have a problem with my order receipt, wa_receipt.php. I got everything to look the way on want it to, but I am frustrated to no end that I cannot see the customers billing and address information on the email order receipt. Everything else displays beautifully except for the address information. I have attached a copy of my wa_receipt.php for your review. I would love to get this completed and fixed before the New Years holiday starts.

It looks like the receipt was using a different form ID than the page had.... I updated the references

from:
CheckoutWizard_NewFromBlank_Default

to:
CheckoutWizard_Sharp_Default

I think that should fix it.
