Email not sending

I've got a login page that loads a form if the user is logged in and doesn't have the required permissions to access the requested url. Everything is working correctly, other than the email. I'm using universal email in other places on the site and the emails are sending, so I know my smtp and port settings are correct. I'm attaching the page to this post for review. Any idea what I did wrong here?

Change your authenticate user server behavior trigger so that it uses the login button press instead of any form submit. It is probably preventing the email from sending because it is trying and failing login before the email can be sent... either that or just move the email code above the login code.

I tried both of your suggestions, but it's still not working. I"m getting a 500 server error now though instead of it just reloading the page.

The website encountered an error while retrieving http://www.vinechurch.net/login.php?accesscheck=%2Falf%2Fcms%2Fuser_management%2Fusers.php%3F. It may be down for maintenance or configured incorrectly.

The code looks correct to me, but it can sometimes be something small. Can I get FTP access to debug it?

I just realized that those credentials wouldn't work for that anyway, because it uses the username as the email address. I'll set you up an account so that you can login and see if you get an email or not.
