Page with Framework Builder Theme Not Displaying

I create a dwt theme, applied it to a blank php page and the page loads and displays without errors. I create a Framework Builder Theme from that dwt and it completed with no errors. When I apply the Framework Builder theme to the existing php page that has the dwt applied and I launch the page, I am getting the errors below: (see test page http://loyalneighbor.com/test.php) What do you think is wrong?

Fatal error: require() [function.require]: Failed opening required 'menu.dwt.php' (include_path='.:/usr/local/php5_3/lib/php') in /home/content/05/11651305/html/loyalneighbor/test.php on line 30

Maybe a bad path the the menu.php page on the page? If you attach a copy of the page I can take a look.

On line 29 the file is referred to as: menu.dwt.php

but the actual file name is menu.dwt

Since the page has php code on it, I'd recommend renaming the file to : Templates/menu.dwt.php
