Insert Form, File Upload, Server Validation, Confirmation Page - what is the correct order for this process?

My scenario...

1) An Insert Form (to database) with an image field
2) A confirmation page with WA Server Validation on it redirecting back to the Insert Form if there's a problem

I just can't get this to work. :(

It occurred to me that doing the validation AFTER the Insert was non-sensical however when I move the validation to the actual Insert Form page I get browser errors - "The page isn't redirecting properly" in Firefox, "This web page has a redirect loop - ERR_TOO_MANY_REDIRECTS" in Chrome and "Page cannot be displayed" in IE.

The File Manipulation > Upload Files behaviour STILL inserts after the INSERT code, which I thought was fixed.

Server Validation, File Upload, and Insert Record server behaviors must all be applied to the same page, in that order, and that page should be the action page of the form.
