Universal Email attachments

I've created an employment form with a browse to submit resume attachment feature. For some reason the attachment is not coming through. Can you please tell me what I've missed? I've done this many times before but for some reason this one is not working. Files have been attached!
Thanks,
Jackie

You have to change the form tag and add: enctype="multipart/form-data"

Hi Ray,
I see that the form name is form1 which is a duplicate within this site, form1 is already being used on the contact us form. Is that part of the problem that it should be changed to "form2"???? Also, where would I update the enctype= multipart/form-data It's strange I have used this feature many times and have never had a problem. I'm sure i just missed a step somewhere.
Thanks for the help!

Just add the enctype to the <form> tag. The form name doesn't matter.
