Custom header on blank.php

I've added a header image to the blank.php page in my universal email element. The problem is that it shows in design but when the form submits, the image is missing. I'm thinkin it's a directory issue but not sure.

Any guidance would be greatly appreciated. I've attempted to add the image to the templates folder along with the blank.php file.

for the header image src, make sure you are using a full URL to your site:

<img src="http://www.yoursite.com/images/header.img" />

also, most email clients won't show images by default, when you get the email, you will need to tell the client to download the images.
