how to I generate an error message. Right now I submit and get a blank page

I need to find out what my problem is on my contact page. I fill it out and submit. It goes to a blank page and doesn't send the email. The page is http://interprimus.com/contactPage.php. I left you the ftp info if you need it.

a blank [page means that error reporting is turned off.

I added the following code at line 1 to turn error reporting on:

<?php
error_reporting(E_ALL);
ini_set('display_errors','on');
?>

the error indicates that the webassist/email/templates/blank_3.php file is not uploaded.

make sure to upload the entire webassist/email/ folder.
