Email from form not sending

Just reviewing some pages and noticed that one of the emails to be sent when a form submitted doesn't look like it is sending.

There are two on the page - one to the site owners with the form information which is working.

But the email notification that is supposed to go to the person filling in the form to the email address they enter is not sending.

I have attached the page here, with more details below.

you have set the goto redirect in both instances.

in the first instance the redirect is set at line 115:
$GoToPage = "";

this redirect happens before the second UE instance can be triggered which starts at line 193.

remove the redirect at line 115:
$GoToPage = "";

so that the second instance of UE can be triggered.
