Getter Error mm sitetoolsactivate is not a function

When I try to click finish in the CSS Form Builder: Add Form, I get this message so it won't let me save it.

Any idea what the cause is?

UPDATE:
And now I'm seeing a new message, see 2nd attachment. Plus, when I try to Create New Form, the window opens and immediately closes so now I can't even get to the place where the original message appeared. Note that this was AFTER I rebooted my machine.

What is your OS?

What version of DW are you using?

What extensions, including the version numbers, are installed?

Have you tried the following:
reset the workspace layout:
go to Window -> Work Space layout -> Reset '<your layout name>'

Then close Dreamweaver and Delete the Dreamweaver Cache file from the users configuration folder:

Hey! Here's the info:

- win 7 professional
- dreamweaver 5.5
- I just downloaded all of the extensions from my shopping cart a couple weeks ago. Confirmed their versions are the latest
- this error just started happening. I could open and implement any of your extension options, then at 430am these errors started appearing. I've rebooted as well. And the error appears immediately upon opening DW now. Every time
- have tried resetting the workspace and clearing the cache w no change

Try uninstalling Data Bridge and installing it again in the extension manager.

After installing the extensions, open Dreamweaver, this should start the WebAssist installer. If the WebAssist installer does not start, you may need to open a PHP page to force it to launch.

after the WebAssist installer completes, make sure to restart Dreamweaver.


If you still have a problem, please list the extensions that are installed and the version numbers, just saying they are current does not really tell me enough.

Ok. I'll do this again when I get to my computer.

If it helps, the installed versions are the version numbers that are listed in my acct. Just can't grab them on my phone.
