looks like you applied your template to all the checkout pages:
checkout.php
confirm_cart.php
confirm.php

the template should only be applied to the checkout.php page.

the other 2:
confirm_cart.php
confirm.php

are loaded through ajax as part of the checkout page, so should not have the template applied to them.
