Custom pages in admin CMS

I created a template page and pages from template for internal communication to be accessed from admin_cms
I modified the pages to be my new left nav and I must have broke the section that allows the page to display. Please see attachment.
OR is it path issue since my internal notes pages are in the InternalNotes directory rather than admin_cms?

I also have issue with my customized left nav which is detailed in the doc as well.
