Cannot get Login to work

Running DW CS6 on OSX 10.9.5, Data Bridge 1.2.1

I'm attaching a zip (archive.zip) of the files Security Assist created to make a test login suite of php pages. I'm having trouble getting the login to open the "data_pa.php" - it simply returns to itself after I fill in the parameters. Everything else with Security Assist seems to be working - I just am having an issue getting the login to authenticate and open the page I want it to. The "data_page.php" page is restricted using SecurityAssist's Retrict Access with a "Log in success" rule. Can you review and let me know what I'm doing wrong?

Don't use the login success rule, use the Logged Into <your user table table name> rule instead.
