at line 192, change the form action:

action="<?php echo $_SERVER["PHP_SELF"]; ?><?php echo (isset($_SERVER["QUERY_STRING"]) && $_SERVER["QUERY_STRING"] != "")?"?".htmlentites($_SERVER["QUERY_STRING"]):""; ?>"

to:

action="<?php echo $_SERVER["PHP_SELF"]; ?><?php echo (isset($_SERVER["QUERY_STRING"]) && $_SERVER["QUERY_STRING"] != "")?"?".htmlentities($_SERVER["QUERY_STRING"]):""; ?>"

Hi Jason, am I missing something here as they both look like exactly the same piece of code?

Thanks, Dan

in the first one, there is a syntax error on:
htmlentites

it should be:
htmlentities

I have logged this as a bug.
