Log in, Forgot password not working

OK, I tried this morning using Security Assist to set up new Log in, Log out, and Forgot password pages. Several problems:
1) Log in not working. It says my log in and/or password are not valid. Not true. Also, the formatting of the returned page is messed up (text flows beyond the Main content box). See www.gcssepm.login.php. Is the fact that this uses a MySQL connection, whereas the rest of my site uses a MySQLi connection part of the problem? If so I need this page corrected ASAP so our members can log in.
2) Where do I put a link for Log out?
3) Forgot Password: the font for Captcha seems corrupted. Dreamweaver gave errors uploading it so I had to do it using FTP; now when you look at the page, a box prompting you to enter info overlaps the Captcha image and you can't read it.
4) Finally some lines will allow me to apply my own CSS and some will not. I'm just talking about Paragraph-level formatting, but your pages won't accept it.
