Complete newbie failing to connect local database in DW CS6. Help!

When attempting to connect a database to a php file in DW, I am getting the following error message:

HTTP Error Code 404 File not found. Some possible causes of the problem:

1) No test server is operating on the server computer.
2) The testing server specified for this site does not map the URL http://localhost/actionbay.net/_mmServerScripts/MMHTTPDB.php. Verify that the URL prefix is mapped to the root folder of the site.

I am not sure what I am missing. Please see screenshots.

judging by the error, you have set the WEB URL in the testing server settings to:
http://localhost/actionbay.net

but the site files are in the c:/xampp/htdocs/forms-testing folder, that folder would map to the address:
http://localhost/forms-testing

that is the address you should use din the testing Server settings for the web url
