helperphp.php error

I've made a form that is to insert data into a MySQL database as well as upload a file to my server with one click of a submit button but I get an error:

Notice: Undefined index: FileName in /Library/WebServer/Documents/FDRC/webassist/file_manipulation/helperphp.php on line 72

I am attaching my code. Any help is appreciated.

The File Upload code is after the insert behavior code, move the insert behavior code so it is after the file upload.

helperphp.php error

I've rebuilt the page (see new code attached) and still get the same helperphp.php error. The form updates the database except for the field to hold the upload's filename (remains blank) and the file does NOT upload.

What am I doing wrong??

I've re-positioned the code as suggested. Same error.

edit the form tag using the property inspector, and set the encoding type to mutipart/Form-Data

Production Server - Same error - Urgent!

I just uploaded all the new admin (limited access) files to our production (public) server and am now getting the same error:

Parse error: syntax error, unexpected '[' in /var/www/html/webassist/file_manipulation/helperphp.php on line 74

All these pages work perfectly on localhost. What could be causing this error on the public server and where do I trouble shoot?
