Image upload troubleshooting

I've been using webassist extensions for many years and am very familiar with the file manipulation upload wizard. I need help trouble shooting an image upload problem that has me totally stumped. When modifying an existing record, I can't get the page to upload a new image. The insert page uploads just fine.

I've made sure the enctype="multipart/formdata" is in the form tag.
I've compared the insert to the upload page.
It doesn't appear to be a file size issue (Even extremely small jpgs fail to upload.
It's not a permissions problem with the target folder (as it works just fine on the insert page).
I've inserted the error reporting code as indicated on other threads.

I've attached the problem page (incidently, the other two table data assist forms fail to work on the upload pages as well), as well as the working insert page. I'll post FTP information in a private message in case you want to look at it directly on the server.

so lets use the insert page to troubleshoot.

to troubleshoot upload issues:

1) turn on error reporting, add this code at line 1:

<?php error_reporting(E_ALL); ini_set('display_errors','on'); ?>

2) add debug info to the page body. open the bindings panel, expand the WA Uploaded files Global status group and add the debug info binding to the page.

3) prevent the insert from redirecting:

change:
$WA_redirectURL = "tbNews_results.php";

to:
$WA_redirectURL = "";

4) write the files array to the page:

<?php
if(isset($_FILES)) {
var_dump($_FILES);
}
?>

post back the results after trying an upload.

That's a little different than my error reporting set up on the upload page. Should I do exactly the same to the upload page?

this in the debug info:

statusCode : 1

and:
clientFileName : BanksWhitePigeon2014Aerial_Facilities-Panel.jpg

means the file is being uploaded. the upload is working.

So why would I be getting different results? I've tried both on my local test server and on the remote server. I've tried several different jpgs. I've tried a GIF. No matter what I do, the file fails to upload. Last report was this:

Status object: WA_UploadResult1
statusCode : 0
errorMessage : No file uploaded
clientFileName :
fileExtension :
serverFileName : Sapphire Square.jpg
serverSimpleFileName :
serverDirectory :
contentType :
fileWasOverwritten :
fileWasSkipped :
fileWasRenamed :
fileSize : -1
isImage :
imageWidth : -1
imageHeight : -1
WA_UploadResult1_1:
clientFileName :
fileExtension :
serverFileName : Sapphire Square.jpg
serverSimpleFileName :
serverDirectory :
contentType :
fileSize : -1
imageWidth : -1
imageHeight : -1
statusCode : 0
errorMessage : No file uploaded
WA_UploadResult1_2:
clientFileName :
fileExtension :
serverFileName : Sapphire Square.jpg
serverSimpleFileName :
serverDirectory :
contentType :
fileSize : -1
imageWidth : -1
imageHeight : -1
statusCode : 0
errorMessage : No file uploaded
array(0) { }

Without being able to reproduce, I cant really say.

what OS are you using?

and Browser? could be a browser issue.

Browser issue hadn't occurred to me but that appears to be the culprit. Using Chrome 36 on Mac. Safari 5.1 appears to work OK. Any direction on troubleshooting the browser issue? My only guess would be some new overzealous security feature that isn't allowing the browser to upload a file?

I should say, I use chrome all the time and have for years and never run into this before.
