Undefined Index error messages.

Undefined Index error messages.

I have added a form that emails the contents but when I submit the form I get a number of error messages:

It might have something to do with the attached javascript error that keeps poping up in Dreamweaver when I'm working on this page.

The email seems to send OK.

For the errors:
Delete the WebAssist/email/mail_php.php and mailformatting_php.php files from the site, the edit the UE server behavior to recreate them.

For the JS error:
attached is a copy of the Shared\Controls\Scripts\WATrigger.js file

locate the users configuration folder, and browse to Shared\Controls\Scripts\ and paste the attached file there.
