Only able to add one new user, then get errors

This is a new one on me - I have a standard registration page to add new users. I added the first record via phpMyAdmin. And it lets me add one new user using the form on the page. But if I go to add another new user, I get errors about the Email and Username already being in use - even if they are definitely not duplicates of other users.

Where this is an insert, in the unique database value validation, set the ID value to 0

Thanks Jason - can I change that in the existing code / behaviour, or is that something I should have done in the set up options?

You can change it in the Server Validations server behavior.
