Adding CAPTCHA results in unknown Warning

I have added CAPTCHA to an existing form like I have done many times. In this instance I am getting this warning at the top of the page:

Warning: session_start() [function.session-start]: Cannot send session cache limiter - headers already sent (output started at /home/mominc/public_html/contact_us.php:3) in /home/mominc/public_html/WA_ValidationToolkit/WAVT_ValidatedForm_PHP.php on line 3

If you would like to see the form it is located here:
contact_us.php

I have no idea what this warning means or how to get rid of it.

If anyone can provide any assistance I would appreciate it. Thanks!

This basically means that there is output to the page at or around line 3:
output started at /home/mominc/public_html/contact_us.php:3

look for a blank line or other characters that may be output to the page on line 2 or 3.
