Can't create the form. Popup asking for default values for all of my text boxes. Why?

I keep getting a popup that wants me to set default values for all of the fields that I created. I don't want default values. how do I get around this and why is it happening?

what version of data Bridge is installed?

make sure you have the latest version, 1.1.8. if you have a previous version installed, uninstall it from the extension manager and reinstall it.
