CSS Form Builder Installation Error

Hello Ray,
When I purchased CSS Form Builder, I installed it without uninstalling the Validation Tool Kit. Later, I uninstalled the Validation Tool kit and the CSS Form Builder. I then downloaded the CSS form builder and installed it. When I tried to Insert form using CSS form builder, I received the following Error:

While execting onLoad in WAFB_AddForm.htm, the following Javascript error(s) occurred:
In file "WAFB_AddForm": initializeUI is not defined.

I later uninstalled the CSS Form Builder and reboot the computer and re-installed the CSS Form Builder. I then tried to enter the CSS form builder by choosing Insert > WebAssist >CSS form builder > Insert Form, I received the same error as above.

Kindly help me

Try deleting the Dreamweaver Cache file from the users configuration folder. more Details on this can be found in the Common Installation Issues PDF that is linked on the Right hand side of the Download center page:
