Ok I admit it. I've owned Dynamic Dropdowns for 3yrs now and never even tried it. I've got it installed and yet I'm a little lost.

I'd like to make a dynamic (MySQL) supplied web form that provides form options related to previous form selections.

Example... If I have a form that offers Beverages or Fruit, then if I pick fruit I'd get a selection of Apples, Oranges, and Bananas, but if I chose Beverages, I'd get a choice of Coffee, Tea or Milk.

Is there an example for such?

I think you will find the sample application included in the Help to suit your needs. index.htm#wa_dd_08.htm

By referring to the Help and the Getting Started Guide you should be able to progress through the tool to implement this functionality. You can find all this from the Support link in the main navigation of the website.
