Web to Lead Configuration (Velocify)

I'm trying to use CSS Form Builder to build a form that will submit the data to Leads360 (now Velocify). They have a short tutorial but it's not very helpful. Velocify provides us with a unique URL/ ID for each campaign. The tutorial can be found here:

I've used CSS Form Builder to send form data via email and that's quite easy. How do I get it to send the data to Velocify instead of an email?

Set the action of the form to use the address that they list in the HTTP POST example.
